An AlayaCare alternative for home infusion nursing agencies.

AlayaCare is a broad home-based care platform with an infusion offering for pharmacies and agencies. Paloma is built from the daily work of a home infusion nursing agency, publishes its price, and lets you start on your own. This page sets out who each is built for, how each is bought and what each covers, so you can decide which fits your agency.

In short

Which one to look at first.

Look at AlayaCare first if…

You are a specialty pharmacy coordinating a network of subcontracted nursing agencies, or a multi-state, multi-line organisation that needs payroll, electronic visit verification and a family portal alongside infusion. AlayaCare lists all of these.

Look at Paloma first if…

You are the nursing agency: you take infusion referrals from specialty pharmacies, send nurses, return documentation and invoice the pharmacy. You want to read the price, open a working demo without a sales call, and start on your own.

Paloma’s current commercial focus is home infusion and specialty pharmacy nursing. Paloma Cares is a broader healthcare operations platform, with Medicare-certified home health coming next and Paloma Talent providing workforce capabilities for eligible subscribers.

The decision

It is less about features than about which side of the referral you sit on.

Home infusion has two operators in every case: the pharmacy that dispenses the drug and the agency that sends the nurse. Software for this market has to choose whose day it is organised around.

AlayaCare describes its infusion product as “one platform to bring pharmacy and agency together”, built for “home infusion providers, specialty pharmacies, and the broader infusion industry”. It describes a workflow in which a pharmacy sends, receives and tracks referrals across its subcontractor network. That is a network view, and it suits an organisation large enough to run the network.

Paloma is run by the agency. The pharmacy’s order governs every visit, the nurse’s documentation returns to the pharmacy in the form that pharmacy requires, and the pharmacy’s invoice is built from reviewed visits. An agency that serves several pharmacies keeps each pharmacy’s requirements, bill-to details and contract rates on that pharmacy’s record.

Home infusion and specialty pharmacy nursing are Paloma’s current focus, where its workflows are built deeply around orders, nursing visits, pharmacy requirements, documentation, staffing and billing. Paloma is a broader operations platform for care delivered beyond the hospital, and it is expanding one service line at a time rather than launching every line at once: Medicare-certified home health is next.

The second difference is how you buy. One is bought through a demo, a proposal and a scoped implementation. The other publishes its price and lets you create your agency yourself.

Where AlayaCare’s site is silent on something, this table is silent too; it does not mean AlayaCare lacks it.

Fairly stated

Where AlayaCare is the stronger choice.

You are the pharmacy running the network

If you dispatch referrals across many subcontracted agencies, AlayaCare describes that network workflow. Paloma is run by the nursing agency, and gives each pharmacy partner a portal into its own work with that agency.

You run several lines of business

AlayaCare covers home care, private duty nursing and pediatrics beside infusion, across multiple states and branches. Paloma’s depth today is in infusion nursing and staffing, with Medicare-certified home health next.

You need payroll or state EVV today

AlayaCare lists both. Paloma exports each pay period and can connect to your payroll provider by integration. Paloma supports visit clock-in and clock-out today. State EVV submission is not yet a completed Paloma workflow.

You want a prebuilt integration marketplace

AlayaCare lists a connector, API management and a marketplace. Paloma builds third-party integrations for a customer’s specific needs rather than offering a marketplace.

You submit insurance claims electronically

AlayaCare lists revenue cycle management. In Paloma, billing is pharmacy invoices and UB-04 claims; end-to-end electronic payer claim submission is not yet a completed Paloma workflow.

You want implementation done for you

A scoped implementation project suits an organisation that wants configuration, migration and training delivered as a service.

What Paloma does

Why a nursing agency chooses Paloma.

These describe Paloma. They are not claims about what AlayaCare cannot do.

The price is on the page

One price per agency, monthly or annual, with every Clinical capability included and no per-user charges. You know the cost before you talk to anyone.

You can see it without asking

The demo agency is a working Paloma with synthetic patients, pharmacies, orders, visits and invoices. Follow one order to its invoice on your own time.

You can start this week

Create your agency, sign a Business Associate Agreement and your order form online, pay at checkout, and go through guided setup: staff, pharmacies, patients, first visit.

Built around the pharmacy’s invoice

Only reviewed, signed visits become invoice lines, at the rates in your contract with that pharmacy. Paloma shows every completed visit that has not yet reached an invoice, and why.

Each pharmacy’s own requirements

Documentation requirements, bill-to details and contract rates sit on each pharmacy’s record, so several pharmacies do not mean several processes.

Built by a nurse who has run an agency

Paloma was built by a registered nurse and agency operator, around the records a surveyor, a pharmacy and a payer will ask to see.

The first screen of a patient chart in Paloma Clinical: the patient, allergies and code status, the open items requiring attention, primary diagnosis, current therapy, care team, next visit and recent activity.
The patient chart, as the agency sees it: allergies and code status, what is open, the governing order, the care team, next visit and recent activity. Before you decide

Seven questions to put to both vendors.

Paloma’s answers are below each question. Ask AlayaCare the same ones.

  1. What will we pay in the first twelve months, including setup?Paloma: the price in the table below is what is charged at checkout, monthly or annual. Before you pay, Paloma shows exactly what is charged today, the period it covers and the next charge date.
  2. Does the price change when we add nurses or office staff?Paloma: no. There are no per-user charges.
  3. How long until our first real visit is documented in the system?Paloma: you can create your agency in one sitting; guided setup then takes you through staff, pharmacies, patients and the first visit at your pace.
  4. Can each of our pharmacies get its documentation back the way it requires?Paloma: per-pharmacy documentation requirements sit on the pharmacy’s record, and the reviewed note returns in the format that pharmacy requires.
  5. How is a pharmacy invoice produced, and what stops an unsigned visit from being billed?Paloma: invoice lines come only from reviewed, signed visits at your contract rates.
  6. What happens to our existing data?Paloma: You can move existing agency data into Paloma using bulk import and guided migration workflows. Paloma does not promise to reproduce every record from another system.
  7. What are we committing to, and how do we leave?Paloma: monthly ends by written notice at the end of the billing period; annual is twelve months paid in advance. The terms are on the pricing page.
Limits

What Paloma does not do today.

Paloma’s current billing workflows support its present operational and pharmacy-billing model. End-to-end electronic payer claim submission is not yet a completed Paloma workflow. Paloma does not process payroll itself: it calculates hours, wages and mileage for each pay period and exports them today, and can connect to your payroll provider by integration. Paloma supports visit clock-in and clock-out today. State EVV submission is not yet a completed Paloma workflow. Medicare-certified home health is the next major service line and is not available yet. Paloma supports its current native workflows and can build third-party API integrations for customer-specific operational needs where the external system supports integration.

Common questions

AlayaCare or Paloma: what agencies ask.

Is Paloma a replacement for AlayaCare?

AlayaCare does not publish prices. Its pricing page, read on October 10, 2026, says it uses custom pricing based on organisation size, care models, regions and modules, with a custom quote after a demo. Paloma publishes one price per agency, monthly or annual, on its pricing page.

Can I try either product before buying?

Neither offers a free trial. AlayaCare’s pricing page says it provides tailored demos instead. Paloma has an open demo agency with synthetic data that needs no sign-up and no card, and a guided demo if you want one.

You can move existing agency data into Paloma using bulk import and guided migration workflows. Paloma does not promise to reproduce every record from AlayaCare.
