Paloma Clinical · Specialty pharmacy nursing

Specialty pharmacy nursing software for the agency that delivers the visit.

A specialty pharmacy dispenses the drug and needs a nurse at the patient’s home. Paloma Clinical runs the nursing side of that arrangement: pharmacy-referred patients, nurse competencies, visit documentation returned to the referring pharmacy, and the pharmacy’s invoice.

Built from the daily work of a Texas home infusion agency and its specialty pharmacies.

In short

What Paloma is, and who it is for.

Paloma Clinical is software for nursing agencies that provide infusion and injection nursing on behalf of specialty pharmacies. It manages the patients a pharmacy refers, keeps each nurse’s competencies current, returns visit documentation to the referring pharmacy in the form that pharmacy requires, and invoices the pharmacy from reviewed visits.

It is for agencies whose customers are pharmacies: one pharmacy or many, in one market or across a state. One price per agency covers every Clinical capability.

Paloma’s current commercial focus is home infusion and specialty pharmacy nursing. Paloma Cares is a broader healthcare operations platform, with Medicare-certified home health coming next and Paloma Talent providing workforce capabilities for eligible subscribers.

The problem

Every pharmacy is a customer with its own rules.

Serving specialty pharmacies means meeting someone else’s requirements, visit after visit, and proving that you did.

Each pharmacy wants something different back

One pharmacy wants its own nursing note form. Another wants yours, within a day. A third will not pay an invoice without the note attached.

The order changes and the nurse does not hear

Dose, rate and frequency are revised by the prescriber through the pharmacy. The nurse in the field needs the order that governs today, not last month’s.

The right nurse for the therapy

Not every nurse is competent for every therapy or route. The pharmacy asks who you are sending and whether their file is current.

Coverage where the patient lives

The referral lands in a town where you have no nurse, while the pharmacy is asking several agencies who can cover it.

Invoices built from memory

Rates differ by pharmacy and visit type. An invoice typed from a stack of notes is late, and sometimes wrong.

Corrections and resubmissions

A pharmacy returns a note or disputes a line. Without a record of what was sent and what changed, the correction becomes an argument.

How agencies handle it today

A folder per pharmacy and a great deal of email.

An agency that serves specialty pharmacies usually keeps a folder for each one: its forms, its rate sheet, its contacts, its portal login. Referrals arrive by fax or through the pharmacy’s portal and are re-keyed. The nurse is sent the order and the pharmacy’s form by text or email, completes it on paper or as a PDF, and sends it back. An office coordinator checks it, forwards it to the pharmacy, and adds a line to that pharmacy’s invoice at month end.

Each added pharmacy adds another parallel process. The agency’s ability to grow is limited less by nurses than by how many sets of rules its office can hold in its head.

Paloma puts each pharmacy’s requirements on that pharmacy’s record, so the same workflow serves all of them.

What Paloma does

The nursing service line, end to end.

  1. Pharmacy referral

Upload the packet as it arrives. AI proposes the patient, the drug, dose, route and frequency, and the prescriber; your intake staff confirm what becomes the governing order.

  2. The right nurse

See licence and competency status before assigning a nurse. With Paloma Talent, a referral outside your footprint becomes a search for prospects near the patient, from the chart, without sending patient details.

  3. The visit and the note

The nurse charts against the order that governs today. The DON reviews and signs. The note returns to the pharmacy in the format that pharmacy requires.

  4. The pharmacy’s invoice

Reviewed, signed visits become invoice lines at the rates in your contract with that pharmacy. Statements, resubmissions and payments are recorded against the same visits.

What Paloma keeps straight

The things that go wrong between an agency and a pharmacy.

Which order governs

Concurrent and revised orders are common in infusion. Paloma records which order governs today, and a nurse charts against that one. A dose that does not match the governing order is flagged before the note is signed.

Each pharmacy’s own requirements

Per-pharmacy documentation requirements, bill-to details and contract rates sit on the pharmacy’s record.

Nurse competencies

Licences, certifications and competencies in one file per nurse, with expirations surfaced before they block a visit.

Completed visits that never billed

Paloma shows every completed visit that has not yet reached an invoice, and why.

Corrections that keep the record

A payment or an invoice is corrected, never edited. The original stays, the correction is recorded with who made it and when, and the resubmission carries the corrected file to the pharmacy.

Factored receivables

If you factor pharmacy invoices, statements can carry the factor’s remit-to details and the factor’s copy is recorded.

Pharmacy portal

Your pharmacy partners can see their side of the work.

Your pharmacy partners do not have to wait for an email. An agency administrator can give a pharmacy’s own staff role-based access to that pharmacy’s work with your agency: their patients and reviewed nursing notes for pharmacists, their patients and referrals for case managers, and invoices for billing contacts. A pharmacy sees its own relationship with your agency and nothing else. How the whole chain works →

Find Staff opens beside the chart. The need comes from the current order; your own eligible staff are checked first; with Paloma Talent, the same need expands to prospects near the patient. Prospects’ licences read “not verified” until your agency verifies them. Synthetic patient, fictional prospects.
Fit

Who Paloma is for, and what it does not do yet.

A good fit

Nursing agencies contracted by one or more specialty pharmacies for infusion or injection visits, paid by the pharmacy per visit under a contract rate.

Know before you buy

Paloma returns the reviewed note to the pharmacy in the format that pharmacy requires and gives the pharmacy its own portal view. Billing today is to the pharmacy. End-to-end electronic payer claim submission is not yet a completed Paloma workflow.

Not what Paloma is

Paloma is the nursing agency’s system. It does not dispense, compound or manage drug inventory, and it is not a pharmacy management system.

Common questions

Specialty pharmacy nursing software: what agencies ask.

What is specialty pharmacy nursing software?

It is software for the nursing that accompanies specialty drugs given at home: receiving the pharmacy’s referral and order, assigning a competent nurse, documenting the visit, returning the documentation to the pharmacy and invoicing the pharmacy. Paloma Clinical does this for the nursing agency.

Can Paloma return visit notes in each pharmacy’s own format?

Paloma keeps per-pharmacy documentation requirements on the pharmacy’s record, and the reviewed note returns to the pharmacy in the format that pharmacy requires.

How are pharmacy invoices produced?

From reviewed, signed visits at the rates in your contract with that pharmacy. A visit with an unsigned or incomplete note is shown as not yet billable, with the reason.

Can Paloma help staff a referral outside our service area?

With Paloma Clinical + Talent, Find Staff searches your own eligible staff first and then Paloma Talent prospects near the patient. Prospects are recruiting leads, not staff: their licences read “not verified” until your agency verifies them.

What does Paloma cost?

Paloma Clinical is one price per agency, with every Clinical capability included and no per-user charges. You can pay monthly or annually; annual saves two months. The current prices are in the table on this page and on the pricing page.
