Paloma Clinical · Private duty and skilled nursing

Private duty nursing software for agencies that send nurses, not paperwork.

Schedule nurses, chart visits, get DON review, and keep every nurse’s file current. Paloma Clinical gives a private duty or skilled nursing agency one record for its patients, its visits and its people.

The same Clinical platform home infusion and healthcare staffing agencies run on today.

In short

What Paloma is, and who it is for.

Paloma Clinical is operations software for nursing agencies, including private duty and skilled nursing. It schedules nurses against current orders, lets the nurse chart the visit on a phone, puts every note in front of the Director of Nursing for review, and keeps one current file for every nurse.

It is for agencies whose care is delivered by licensed nurses in the home or the community. One price per agency covers every Clinical capability, with no per-user charges.

Paloma’s current commercial focus is home infusion and specialty pharmacy nursing. Paloma Cares is a broader healthcare operations platform, with Medicare-certified home health coming next and Paloma Talent providing workforce capabilities for eligible subscribers.

The problem

The care is skilled. The office tools are not.

The schedule and the orders disagree

A change in the plan reaches the family and the nurse, but not the calendar.

Notes arrive late, or not at all

Paper notes come back at the end of the week. Nobody reviews them until someone asks.

A lapsed credential on a live case

A licence or CPR card expires while the nurse is still on the schedule.

The nurse has to call the office

Today’s visits, the current order and what the note needs are not in the nurse’s hand.

Review is a formality

Without a queue, clinical review happens when there is time, which means it does not happen.

Survey problems found too late

Gaps surface when the surveyor asks, not when they could still be fixed.

How agencies handle it today

A calendar, a binder per patient and a binder per nurse.

Private duty agencies often run on a scheduling calendar, paper or PDF visit notes, and binders: one for each patient in the home and the office, and one for each nurse’s personnel file. The system depends on a coordinator who remembers which binder is missing what.

Paloma replaces the binders with one chart per patient and one file per nurse, and connects both to the schedule, so the question “is this visit covered by a nurse who is cleared to make it?” is answered before the visit.

What Paloma does

Four parts of the work, one record.

  1. Schedule nurses

Turn current orders into single visits or multi-day courses. Find nearby eligible nurses and check staff readiness before assignment.

  2. Chart visits

The nurse’s phone shows today’s visits and a guided note that saves as the nurse works. A visit in progress stays on top until the nurse clocks out.

  3. DON review

A review queue for every submitted note. Notes can be returned to the nurse for correction, and the nurse sees exactly which notes still need attention.

  4. Every nurse’s file current

Licences, certifications and competencies in one file, with expirations surfaced before they block a visit. Nurses can upload replacements from a phone.

Fit

Who Paloma is for, and where to look closely first.

A good fit

Private duty and skilled nursing agencies whose nurses make visits under a physician’s orders, and agencies that combine that work with infusion nursing or staffing.

Look closely first

Paloma’s guided visit note is built around skilled nursing visits and is deepest for infusion. If your work is long hourly shifts with flowsheet charting, open the demo agency and judge the documentation against your own before deciding.

Not what Paloma is

Paloma does not offer non-medical personal care or Medicaid personal-care billing today. Paloma supports visit clock-in and clock-out today. State EVV submission is not yet a completed Paloma workflow.
