Referral → order

Order → visits
The governing order sets the treatment course and the staffing need. A revised order is a new order, never an edit, so every visit can show which order it was given under.
Visit → documentation
The nurse charts against the current order: dose given, route, site, vitals, reactions. The DON reviews and signs. The note returns to the pharmacy in the format that pharmacy requires.
Reviewed visit → invoice
Only reviewed, signed visits become invoice lines, at the rates in your contract with that pharmacy. Statements, resubmissions and payments are recorded against the same visits.

Which order governs
Concurrent and revised orders are common in infusion. Paloma records which order governs today, and a nurse charts against that one. A dose that does not match the governing order is flagged before the note is signed.
